PCC Services Inc is a growing Nonprofit organization that supports developmentally disabled teens and adults with coordinating services needed as they transition into adulthood and independence. 

Out staff completes a comprehensive patient-centered needs assessment and includes a medical physical to ensure all individualized needs are addressed. PCC Services Inc then creates a comprehensive plan for each patient that can be used with the Primary Care Provider of their choice.

My child is turning 18 soon, what medical benefits will they receive as an adult?

Transitioning from teen to adulthood will not change your child's medical benefits, however most teens will transition from a pediatrician to a Primary Medical Provider for adults. PCC Services, Inc can help you in this transition process with our readiness assessment that is given to the patient, parent and caregivers in order to understand individual needs and gaps in care that need to be addressed and prepare patients for this transition stage of life. PCC Services can also help you locate a Primary Care Provider if needed.

What community services are available for my teen or young adult child who has a developmental disability?

Every individual has their own set of qualification criteria for services within their community. PCC Services, Inc. will meet with you and your loved one, complete a risk assessment, identify individual needs for your specific case and connect you to resources within the community you live in. These can include housing, daily living support needs, financial support, job search and training support and more!

How much will my copayment be for Home medical visits and Telehealth visits?

Insurance coverage varies by the plan. Call PCC Services, Inc to see what your co-payment will be based off of your insurance carrier. We also have funding to help those who do not have insurance and have low income needs. At PCC Services, Inc we want to ensure that everyone gets the help they need to live the most independent life possible within their community!

My loved one lives in a group home, will your provider go to them in an assisted living setting?

Yes! Our medical providers and case managers complete in-home assessments for those who are living in independent living, assisted living and group home settings.
