Our Medical Providers and Case Managers invite you to become a part of our growing and supportive community. We are a nonprofit organization who specializes in interdisciplinary and comprehensive healthcare coordination of community services for individuals with developmental disabilities (ages 13 and older) as well as elderly and aging populations within community settings. We believe everyone should have access to quality healthcare and it is our mission to break down barriers keeping populations with healthcare disparities from the care they deserve.
